If you have an appointment with our Pediatric Dentist, Please remember to bring: a reason for referral, a copy of x-rays, insurance card and a picture ID. Please do not give your child any heavy meals, or milk 2 hrs before appointment.
For Adults: Please remember to take all necessary medications, and have a light meal or snack before any treatment. Please inform us of any Pre-medications needed before appointment.
